Health Sciences Program Students

Health Sciences program students will use the paper version of the Health Science Programs Medical Evaluation Physical and Immunization Review Packet [pdf] for their physical. This packet will now also be scanned into your Electronic Health Record.

RedTail Student Athletes

Student athletes will use the paper version of the Preparticipation Physical Evaluation Forms for sports physical on or off campus. The following forms are required to be printed and completed:

The nurse practitioner and licensed mental health counselor are also available via Tele Health.
