Appointments

All students registered at Hawkeye Community College may use Hawkeye Student Health Clinic services. Care and services are provided at no cost or minimal cost.

Appointments are required: To schedule an appointment with the nurse practitioner or licensed mental health counselor please call, email, or simply walk-in to the clinic.

Before Your Appointment

Upon scheduling your appointment you will receive an account entry link to "Elation EHR" sent to your Hawkeye email. Please complete the following forms before your first visit:

  • Demographics
  • Medical history intake questions
  • Health screening questionnaires
  • Consents
  • Additional forms as needed or requested

What to Bring to Your Appointment

When visiting the Student Health Clinic, please bring the following:

  1. Your Hawkeye student ID

  2. Your health insurance card if available.

    • Health insurance is not required to receive care and services are no cost or minimal cost, however, it's helpful to have your health insurance information.
  3. RedTail Athletes and Health Sciences students will need to print and bring the additional pre-appointment forms listed below.

Health Sciences Students Pre-Appointment Forms

The Health Sciences program students will use the paper version of the Health Science Programs Medical Evaluation Physical and Immunization Review (MEPIR) [pdf] for their physical. Please print this packet, complete the patient portions prior to your physical, and bring it with you to your appointment at the Student Health Clinic or to your personal doctor. These forms will be scanned into your Electronic Health Record.

RedTail Athletes Pre-Appointment Forms

Forms for your Preparticipation Physical Evaluation will be sent to you electronically after your appointment at the Student Health Clinic is scheduled. Please complete your forms prior to your appointment.

If you will be going to an off-campus clinic for your physical, print the following forms and take them with you to your physical:

All RedTail athletes will have an Electronic Health Record. Physicals scheduled and performed in the Student Health Clinic will be entered in the student athlete's Electronic Health Record. A printed version can be shared by the patient or at the patient's designation.

If campus is closed, the Student Health Clinic is also closed. This includes weather-related closings. See scheduled college closings.
