Today's COVID-19 Vaccine Clinic is open to Hawkeye employees and their spouse, significant other, or children over the age of 18; and student employees.

Before your appointment review the Moderna Vaccine Fact Sheet and download, print, and sign the COVID-19 Vaccine Consent form [pdf] and bring it with you to your appointment.
The second dose of the Moderna vaccine will be available on campus to those participating in this clinic in four weeks on Thursday, May 13.
We are grateful for the opportunity to provide this vaccination clinic and continue to encourage everyone to get vaccinated as they are able to do so. Additional vaccine locations can be found through the Vaccinate Iowa website.
Our best hope for ending the pandemic is through vaccination. If you have not yet been vaccinated, I hope you can take advantage of this opportunity.
