READ BEFORE SUBMITTING: I understand running a race is a potentially hazardous activity which could cause injury or death. I attest I am physically fit and have trained for competition in the Scarecrow Scamper and assume all risks associated with running in this event. If, however, as a result of running this race I become injured, I, for myself and anyone entitled to act on my behalf, waive and release Hawkeye Community College, Cedar Valley Arboretum & Botanic Gardens and anyone affiliated with the Scarecrow Scamper from all claims or liabilities of any kind arising out of my participation in this event, even though that liability may arise out of negligence or carelessness on the part of the persons named in this waiver.
I grant permission to all the foregoing to use my photographs or any other record of this event for any legitimate purpose. I, the undersigned have read the above waiver/release and understand I have given up substantial rights by submitting it.
By submitting this form, I understand that I have conditionally agreed to the terms of above until I have signed the registration form the day of the race. I also understand that if I am under the age of 18 I must have a parent or guardian present the day of the race to sign the registration form.
