2012-13 Missing Documents and Information Needed for Your Financial Aid File


Bachelor / Master / Doctorate’s Degree

The answers you gave on Items #28, #30 and/or #47 on your FAFSA indicate that you have or will have a four-year bachelor’s degree (BA or BS) by July 1, 2012. This may affect your eligibility for the Pell Grant.

Please complete the Bachelor / Master / Doctorate Degree Statement form [pdf] and submit it to the Financial Aid office.

Bad / Ended Major (Academic Program)

The Department of Education requires students to be degree seeking and accepted in an eligible academic program to receive financial aid.

To receive financial aid, you must have applied for admission and have been accepted into an eligible program.

If you are not pursuing a degree or diploma from Hawkeye Community College, you should contact your home institution about financial aid.

Citizenship Status

The Department of Education reports that there may be a problem with your citizenship status. Please refer to your Student Aid Report (SAR) for information.

Documents needed:

  • If you are a U.S. born citizen, we need a copy of your birth certificate.

  • If you are a national or an eligible non-citizen, we need a copy of any document that proves your status, such as a permanent resident card (alien or green card) or a certificate of naturalization.

Please bring the necessary document to the Financial Aid office.

Emancipated Minor

You stated that you are/were an emancipated minor.

Document needed: official court document verifying that you are or were an emancipated minor immediately before you reached the age of being an adult in your state. Document needed if you were born on or after January 1, 1988.

If you submitted these documents to us during a previous year, or have questions, please contact our office.

The court making this decision must be located in your state of legal residence at the time the court’s decision was issued.

If you are unable to document your dependency status based on an emancipated minor document, then you must correct your Student Aid Report (SAR) to include parental information and signature.

FAFSA Rejected

The Department of Education was unable to complete the processing of your FAFSA. The reason is usually due to incomplete information, name not matching with Social Security Administration, or missing signatures on your FAFSA. Please refer to your Student Aid Report (SAR) for information or call 1-800-433-3243.

What you need to do: Correct your SAR and submit it to the federal processor. Once we receive your revised results we can continue to work on your financial aid file.

Grant or Perkins Loan Overpayment

The National Student Loan Data System (NSLDS) reports that you have an overpayment of financial aid that needs to be repaid. Refer to your Student Aid Report (SAR) for information.

If you do not owe an overpayment, please give us an official document to verify the status of the overpayment(s).

If you owe an overpayment, we are unable to process your financial aid.

High School Degree

On your FAFSA you either did not answer the question about having a high school degree or GED or your answer was number 4 (“Other”).

If you do not have a high school degree or GED, you are NOT eligible for financial aid.

To receive federal and state financial aid you must meet the eligibility requirements.

If you are currently a high school student, you are not eligible to receive financial aid.

Iraq and Afghanistan Service Grant

The Department of Defense (DOD) has indicated to us that you may be eligible for this grant. In order to verify your qualifications, please set up an appointment to meet with a financial aid advisor.

Legal Guardianship

You stated that you are or were in legal guardianship.

Document needed: official court document verifying that you are or were in legal guardianship immediately before you reached the age of being an adult in your state. This document is needed If you were born on or after January 1, 1989.

If you submitted these documents to us during a previous year, or have questions, please contact our office.

The court making this decision must be located in your state of legal residence at the time the court’s decision was issued.

If you are unable to document your dependency status based on a legal guardianship document, then you must correct your Student Aid Report (SAR) to include parental information and signature.

Loan Bankruptcy

The National Student Loan Data System (NSLDS) reports you have one or more federal student loans in active bankruptcy. You must set up an appointment with a financial aid advisor to resolve this issue.

To review your federal loan history, go to www.nslds.ed.gov.

Loan Default

The Department of Education reports that you may be in default on a student loan. Go to www.nslds.ed.gov to determine what loan(s) are in default.

If you are not in default, please give us an official document from your loan holder reflecting that your loan(s) is in good standing.

If you are in default, we are unable to process your financial aid. Please contact your lender(s) to resolve this status.

Loan Discharge

The National Student Loan Data System (NSLDS) reports that one or more of your federal student loans has been discharged.

Document needed: Physicians Certification form [pdf]. Return the completed form to the Financial Aid office.

To review your federal loan history, go to www.nslds.ed.gov.

Loan Entrance Counseling

In order to receive your Federal Direct Stafford Loan funds for 2012-2013 academic year, you must complete loan entrance counseling.

Loan History Issue

The National Student Loan Data System (NSLDS) was unable to match your record due to missing information or system problems. Please contact our office about this issue.

Marital Change

You have changed your (or your parents’) marital status and/or date on FAFSA Item #16, #17, #58 and/or #59 to a new response. The answers to these questions must be your (or your parents’) status as of the date you signed and submitted your initial FAFSA.

Please give us a signed written statement indicating your (or your parents’) marital status, date of marital change, and whether or not you included your (or your parents’) current/ex spouse’s 2011 financial information on the FAFSA.

Master Promissory Note

In order to receive your Federal Direct Stafford Loan funds for 2012-2013 academic year, you must sign a master promissory note.

More Info Needed

After processing your submitted paperwork, we found discrepancies that need to be resolved before we may continue reviewing your file. You should have received a worksheet in the mail with specific information regarding this. Please contact the Financial Aid office for more details.

Orphan or Ward-Of-The-Court

You stated that you are or were an orphan or ward-of-the-court, or were in foster care at any time since you turned age 13.

Document needed: Official Court Document verifying your legal orphan, ward-of-the-court situation, or foster care at any time since you turned 13 even if you are no longer a dependent or ward of the court as of today. This document is needed if you were born on or after January 1, 1989.

If you submitted these documents to us during a previous year, or have questions, please contact our office.

Please understand that the Department of Education does not consider a ward-of-the-court based on being incarcerated or if you were in foster care/ward of the court only prior to age 13. If this fits your situation, we are unable to document your dependency status based on a foster care/ward-of-the-court document. This means that you must correct your Student Aid Report (SAR) to include parental information and signature.

Parent 2011 Federal Tax Return Transcript

Document needed: signed copy of your parent(s) 2011 Federal Tax Return Transcript. We are no longer able to accept the actual tax forms; we must have IRS issued tax return transcripts.

You can request your tax return transcript on the IRS website.

Be sure that the tax return transcripts are of the parent / stepparent(s) that were used on your FAFSA.

If your parent(s) are not required to file a 2011 Federal Tax Return, please indicate so on the Verification Worksheet – Section C. Please complete the appropriate Verification Worksheet above.

Otherwise, you may utilize the IRS Data Retrieval Tool by making a correction to your FAFSA.

Selective Service Registration

The Department of Education reports that there may be a problem with your selective service registration. Please refer to your Student Aid Report (SAR) for information.

You may register on the web or at any U.S. Post Office.

Documents needed:

  • Once you have received your Selective Service Card, send a copy to our office.

  • If you have never registered with Selective Service before turning 26 years old, please contact Selective Service to obtain documentation regarding your registration status.

    Provide to us this document along with a detailed signed letter, and any supporting documentation, to explain your situation between the ages of 18 to 26.

Social Security Number

The Department of Education reports that there may be a problem with your social security number, birth name, and/or birth date listed on your FAFSA. Please refer to your Student Aid Report (SAR) for information.

If it appears that your social security number, birth name, and/or birth date are correct on your SAR, you should call the Social Security Administration at 1-800-772-1213 to verify and update their records.

Documents needed: a copy of your social security card and birth certificate (original documents) to our office so we may continue to work on your financial aid.

Student 2011 Federal Tax Return Transcript

Document needed: signed copy of your (and your spouse’s, if married) 2011 Federal Tax Return Transcript. We are no longer able to accept the actual tax form; we must have IRS issued tax return transcripts.

You can request your tax return transcript on the IRS website.

If you (and your spouse, if married) are not required to file a 2011 Federal Taxes, please indicate so on the Verification Worksheet – Section C. Please complete the appropriate Verification Worksheet above.

Otherwise, you may utilize the IRS Data Retrieval Tool by making a correction to your FAFSA.

Student Loan Game Plan

The Student Loan Game Plan is a tool to assist you in borrowing wisely. You are required to complete the Student Loan Game Plan before any Federal Student Loans will be disbursed. You will only need to complete the Student Loan Game Plan once.

Unaccompanied Youth Who is Homeless or At Risk of Being Homeless

You stated that you are homeless or at risk of being homeless.

Document needed: official document verifying that you are or at risk of being homeless as determined by your high school or the director of an emergency shelter or transitional housing funded by the U.S. Department of Housing and Urban Development. This document is needed if you were born on or after January 1, 1989.

If you submitted these documents to us during a previous year, or have questions, please contact our office.

If you are unable to document your dependency status based on these criteria, then you must correct your Student Aid Report (SAR) to include parental information and signature.

Veteran Status

You indicated on your FAFSA that you are a veteran.

Document needed: a copy of your DD214 Form-Member 4 form. This document is needed if you were born on or after January 1, 1989.

If you have submitted your DD214 to the VA Certifying Official in efforts to obtain your Montgomery GI Bill education benefits, please contact the Veterans Affairs Certifying Official to request that we use the already submitted copy.
