Applying for Accommodations

How do I apply for accommodations?

  1. Complete a Student Reasonable Accommodation Request and Release Form. Paper forms are available in Student Services.
  2. Make an appointment with the Special Needs Coordinator to share your disability and accommodations you need while a student at Hawkeye Community College.
  3. Provide current documentation of your disability. This can be brought with you to the initial appointment, mailed to Student Services, or once the release is signed, we can request the documentation for you. For more information on what is considered current documentation, please visit with the Special Needs Coordinator.
  4. After we have met with you and have received your current documentation, the Special Needs Coordinator will review all of the information and determine the accommodations you will be allowed based upon your specific disability.
  5. You will be notified of your eligibility to receive accommodations either in person or through your Hawkeye email. You may pick up copies of your accommodations letter in the Student Services Office. You should share this letter with your instructor(s) during the first two weeks of the semester. It is important for you to talk with your instructor(s) about your learning needs EARLY in the term. Instructors have office hours and often may be reached before or after class to arrange a time to meet. It is your responsibility to keep instructors informed of your needs.

What kind of documentation is needed to receive accommodations?

You will need current documentation from a professional. Depending on the disability, we will need a letter from a qualified professional telling us what the disability is, the testing used to diagnose the disability, and what type of accommodations will work best for you. You may need to be retested for Learning Disabilities. We can provide referrals for these services.

If you will be exiting high school special education services, we encourage you to request your resource teachers to complete a Support for Accommodation Request (SAR) to supplement your accommodations application.

You can come to the Student Services Office where we will give or assist you with the Student Reasonable Accommodation Request and Release Form. Or you can call us at 319-296-4014 and someone will send you the form.
