Documents Required Before an Award May Be Made

Only selected students are required to complete these forms. Our office will contact you if the form(s) are needed.

Dependent Verification Worksheet
To be completed if FAFSA was selected to be verified. Also, include a signed copy of your and your parent 2006 Federal Tax Return
Independent Verification Worksheet
To be completed if FAFSA was selected to be verified. Also, include a signed copy of your and your spouse (if married) 2006 Federal Tax Return
FAFSA Worksheets A and C
To be completed if there is a discrepancy between the untaxed income reported on the FAFSA Worksheets A or C and the Verification Worksheets A or C.
Dependent Low Income Form
To be completed if the income your parent reported on your FAFSA was determined too low to cover basic living expenses
Independent Low Income Form
To be completed if the income you and your spouse (if married) reported on your FAFSA was determined too low to cover basic living expenses
Drug Question Worksheet
To be completed if you either did not answer item #31 on your FAFSA or marked it as "Yes"

Direct Subsidized, Unsubsidized, & Plus Loans

Direct Loan In School Deferment Form
Used to defer loan payments if you are returning to college
Parent PLUS Loan Data Form
If interested in a Parent PLUS Loan, please complete and submit the Loan Data Form to our office. In addition, there is a brochure available in our office with details regarding the PLUS Loan Program.
Direct Loan Request Form
The Award Letter outlines your eligibility for the Federal Direct Stafford Loan Program (Subsidized and/or Unsubsidized Loans). If you wish to borrow for the 2008-2009 Academic Year, you may Accept/Reject Federal Loans on your SOAR account or fill out this form and return it to the Hawkeye Financial Aid Office.
Responsible Borrowing Worksheet
Used with a Responsible Borrowing session for students with high Stafford Loan debt
Summer 2008 Financial Aid Application
Please complete and turn in to the Financial Aid Office to apply for summer financial aid.

Private Loan Forms

These forms are required each school year of all students taking out an Alternative Loan at Hawkeye. The Alternative Loan and Debt Management Worksheets must be completed and submitted to the Hawkeye Financial Aid Office.

Work Study Termination Form
To be completed if a student or supervisor terminates the student’s position in the work-study program
Work-Study Orientation
All new work-study students are required to take the quiz after reading the Student Employment Handbook and Student Employment Soft Skills Training. Please print out the quiz, complete it, and return it to the Financial Aid Office.

Satisfactory Academic Progress (SAP) Forms

Used to apply for a variety of scholarships administered by the Hawkeye Foundation's Office.
Consortium Agreement Information
Contains information on consortium agreements
Consortium Agreement Form (Hawkeye is the Home institution)
Used by Hawkeye students who plan to take classes from another college and wish to include those credit/hours while receiving financial aid from Hawkeye
